Participant Consent, Health & Safety and Risk Acknowledgement
Somatic and yoga therapy interventions invite you to connect to your body through awareness, conscious breathing or mindful movement. This waiver outlines the associated risks and responsibilities associated with participating.
1. Voluntary participation
I understand that participation is voluntary. I may:

Decline any exercise, movement, breathing practice or other activity

Take a rest or break at any time

Stop the session entirely
I understand that I do not need to justify my decision to stop or modify an exercise.
2. My responsibility for my own participation
I understand that online yoga is undertaken without the practitioner being physically present in the same room as me. Therefore I understand I am responsible for:

Providing a suitable and safe enough space to practice

Following instructions within my own capabilities

Stopping or modifying an activity if I experience pain, dizziness, breathlessness, distress or another concerning symptom.
I understand that the practitioner cannot physically observe or assist me in the same way as they could during an in-person session.
3. Assumption of physical and emotional risk
I understand that it is my responsibility to ensure that I am physically and medically cleared to safely participate in yoga-based interventions. This may include consulting with medical professionals to ascertain the appropriateness of these practices for your health.
I understand that participation in therapeutic yoga involves physical activity and that, even when appropriately modified, they may carry risks such as: muscle soreness or strain, aggravation of an existing injury or condition, dizziness or faintness, changes in blood pressure, shortness of breath or other physical injury or effects.
I understand that therapeutic yoga and body-based practices can sometimes bring attention to physical sensations, emotions, memories, thoughts or bodily responses that may feel unexpected or uncomfortable such as: increased emotional awareness, sadness, anxiety or distress, changes in arousal or energy, dissociation or feeling disconnected, temporary feelings of vulnerability or overwhelm.
I understand that no practice is guaranteed to be suitable for every person or every health condition. I understand that I can decline any practice and that I can leave the session at any time. The practitioner will aim to work in a trauma-informed, choice-based and appropriately paced manner. However I understand that the practitioner cannot guarantee that a particular practice will not cause psychological or physical discomfort.
4. Scope and limitations of the sessions
I understand that the sessions are intended to support my wellbeing and, where applicable, compliment my therapeutic support.
I understand that yoga is not a guaranteed treatment for any medical or psychological conditions, that the practitioner cannot guarantee a particular outcome and that the practitioner does not replace my existing support eg psychotherapist, GP, consultant etc. If I am experiencing a medical or psychological emergency, I understand that I should use the appropriate emergency or urgent-care service.
5. Recording of sessions
I understand that sessions will be recorded to provide access for participants who cannot attend live. I understand that I must not make an audio, video, screen or other recording without the practitioners prior permission.
6. My confirmation and consent
By booking a space in a Therapeutic Yoga class with Sally Morrison I confirm that:

I understand that trauma-informed practices may sometimes product unexpected emotional or physiological responses

Data Protection and GDPR Information
Types of information I may hold about you:
(1) Your Name & Address
(2) Your Contact Telephone numbers and Text/Whatsapp Messages (sent/received)
(3) E-mail address and e-mail correspondence (sent/received), including website enquiries
(4) GP contact details
(5) Emergency Contact persons name and phone numbers
(6) Any brief notes that I may make
(7) Data relating to how and when you access my website, including your IP address
This data is collected as part of our work together, in any additional correspondence we might have and (if relevant) through the contact form on my website, the contracting phase of psychotherapy or by booking a yoga class through bookwhen.
I use your data for the following purposes:
(1) To provide therapy and yoga services as agreed (if relevant)
(2) To comply with applicable laws and regulations
(3) To create aggregated statisical data about use of my website.

Should you have any concerns or complaints about the way I hold your data, please contact me via email (sallymorrisontherapy@outlook.co.uk). I will acknowledge your complaint in writing within 30 days and attempt to resolve it within 3 months. If my resolution is not to your satisfaction, you have the right to escalate your complaint to the ICO (ico.org.uk / 0303 123 1113). I will retain records of your complaint for 6 years.
Sallymorrisontherapy.com is hosted by the Wix platform. Any data that you submit via the website will be stored through Wix.com’s data storage servers which are secured and behind a firewall.
I will use your data to contact you as you have indicated in your communication with me. I may also use your email address to include you in communications about news and updates relating to the activity you have expressed interest in. If you'd rather I didn't do this, you can indicate that on the online form, or email me. The data held is never be shared with third parties except for reasons outlined in the confidentiality agreement.
I reserve the right to change this privacy policy whenever I deem necessary to continue to provide a high quality of service.
Data Retention Policy
I will keep the data I hold about you for 5 years following the end of our work together, unless we have agreed otherwise. At the end of the retention period I will securely destroy your data.
You have the right to request a copy of the data held or request any corrections that you feel are warranted. All requests received should be actioned within 28 days. I will check the validity of the request (to the best of my ability) to safeguard your data from misuse. Unless I am informed of changes in the data then you cannot be fully protected under the GDPR, but the data I hold will continue to be held in line with the GDPR.
If you have a concern about how your data is being handled, then you have the right to complain to the ICO (Information Commissioner’s Office).

By submitting any data via the website or communicating with me in a different way, you confirm that you are fully aware of what data is being held and that you are happy for this data to be held in line with the GDPR, UK law and my retention policy.
